Анотація
На примере построения ранней объективной диагностики сахарного диабета второго рода продемонстрирована возможность определения значений внутренних диагностических параметров организма пациента по доступным для измерения клиническим данным, в чем нуждаются доказательная медицина и телемедицина. Задача решена путем построения соответствующей математической модели физиологической системы регуляции углеводного обмена и пересчета по ее алгоритму доступных для измерения гликемических данных перорального теста толерантности к глюкозе, имеющих косвенный интегральный характер, в необходимые для диагностики внутренние параметры инсулинорезистентности тканей.
On the example of construction of early objective diagnostics of the Diabetes mellitus II type it is shown the possibility of determination of the internal diagnostic parameters values of the patient’s organism from the available for measuring clinical data, having indirect integral character, in what evidential medicine and telemedicine need. At present these data are effectively used in intuitional expert medical diagnostics. Its success indicates that, in principle, the so effective extraction by the technical means the latent diagnostic data from the integral laboratory measurings is possible, thus in numeral kind. The task is decided by the construction of the corresponding one-compartmental mathematical model of the physiological system of the carbohydrate exchange regulation and on its algorithm the evaluation the accessible for measuring glycemic data of the glucose tolerance peroral test, having integral character, in necessary for diagnostics internal parameters of insulin-resistance of the tissues. The model offer itself the differential equation of the first order with delayed argument. When itself constructing, the functional-structural method of approach to model with detailing the model of the "black-box" level not in the morphological aspect, but only in functional aspect with partial revealing it’s content was used. It is elucidated that early revealing the latent Diabetes mellitus II type according to internal diagnostic parameters values by effectiveness practically don’t yield to the expert diagnostics, but excel it in possibility to carry out the differential diagnostics of this heterogeneous condition.
