Документ About the influence of health care financing, medical personnel’s qualification and staffing to provide medical care for a patients with a cerebral stroke in Ukraine(ISGT Publishing House, 2020) Mishchenko, Maryna Mykhailovna; Shevchenko, Alexander Sergeevich; Mishchenko, Alexander NykolaevichSurvival of patients with cerebral stroke, both in the acute period (the “therapeutic window” after a stroke is 4.5 hours) and in the long-term (half a year after a stroke), depends on the quality of diagnosis and medical care. Ukraine is one of the countries with a high mortality rate for stroke patients, therefore, measures to improve medical care are relevant: reducing the shortage and qualifications of the neurological service staff, providing medical institutions with diagnostic equipment and medicines, creating more stroke centers, increasing state funding for treating patients.Документ About value and study of the various risk factors for the development of a cerebral stroke(Publishing House "ACCENT", 2020) Ognev, Victor Andreevich; Mishchenko, Marina Mikhailovna; Mishchenko, Alexander Nikolaevich; Shevchenko, Alexander SergeevichRegional registers of strokes, heart attacks, and transient ischemic attacks are compiled in a uniform pattern in different countries of the world since the 1970s. They allow conducting epidemiological studies, studying the risk factors for cerebral strokes, developing and adjusting treatment, prevention and rehabilitation schemes for victims, and conducting prospective cohort studies.Документ Cerebral stroke prevention programs(Barca Academy Publishing, 2020) Shtefan, Liudmyla Vladimirovna; Mishchenko, Marina Mikhailovna; Shevchenko, Alexander Sergeevich; Mishchenko, Alexander NikolaevichCerebral strokes are the second most common cause of premature death in the world after coronary (ischemic) heart disease. A significant part of the risk factors for acute cerebrovascular accidents is - manageable‖, so a significant portion of stroke deaths is preventable. For those experiencing transient ischemic attacks and stroke victims, avoiding these risk factors is equally important. The programs of primary, secondary and tertiary prevention of strokes are implemented by medical institutions, medical and social non-governmental organizations, and educational institutions. The prevention of cardiovascular diseases can be included in continuing education programs, briefings and structured cases on the formation of a healthy lifestyle.Документ Determination of somatic, neurological and psycho-emotional manifestations of the acute and postcovid period in patients with a mild course of COVID-19 in wartime(Wydawnictwo ALUNA, 2023) Fedak, Bogdan S.; Ponomaryov, Volodymyr I.; Ponomaryova, Valeria V.; Kravchenko, Iryna M.; Morozova, Olga G.; Yaroshevskiy, Oleksandr A.; Mishchenko, Oleksandr M.The aim: To analyze the typical symptom complex at the stage of COVID-19 acute phase in the systemic relationship with somatic, psychosomatic, and neurological manifestations. Materials and methods: The collection of primary material was performed by clinical-anamnestic method, laboratory, and sociological examination of patients treated out patiently. Summarizing of the results was performed according to the analysis of 100 completed cases of COVID-19 in patients aged 35-45 years (50 men and 50 women) who had no concomitant chronic pathology, and patients did not receive any vaccine dose before the disease (acute COVID-19) and during the next follow-up period (6 months). Results: The data of the analysis allowed us to make a grounded conclusion about the syndromic heterogeneity of COVID 19 in a standardized patients group with a mild course. Conclusions: the highest number of symptoms in the postcovid period by frequency, polymorphism, and life quality impact was found in the group of patients with subjectively tolerate acute COVID-19 most easily. Patients whose acute episode meets the mild criteria have pronounced neurological and psychoemotional manifestations during the postcovid period.Документ Functional characteristics of the cardiovascular system of patients with ischemic heart disease with obesity(Wydawnictwo ALUNA, 2023) Zlatkina, Vira V.; Nemtsova, Valeriya D.; Fedak, Bogdan S.; Ponomaryov, Volodymyr I.; Zhelezniakova, Natalia M.; Mishchenko, Oleksandr M.; Horban, Dariia V.The aim: To determine the features of the functional characteristics of the cardiovascular system of patients with ischemic heart disease with obesity. Materials and methods: Examined 130 persons (mostly military personnel and persons who were in the zone of active hostilities): 65 patients (the main group, 62,67±8,93 years) with coronary heart disease and obesity and 45 people of the control group (virtually healthy people, randomized by age and sex, 58,76±14,6 years). Results: Coronary heart disease and obesity compared to healthy individuals probably the exceed all values of the functional state of the cardiovascular system: systolic blood pressure (152.72±14.61 and 119.03±7.94 mmHg; p<0.001); diastolic blood pressure (90.74±7.36 and 80.36±6.74 mmHg; p<0.001); end-diastolic volume (103.17±40.84 and 52.48±8.58 mm3; р<0.001); end-systolic volume (47.98±29.92 and 31.47±8.42 mm3; р=0.001); end-diastolic size (4.74±0.81 and 4.12 ± 0.27 cm; р<0.001); end-systolic size (3.34±0.76 and 3.17±0.59 cm; р=0.014). Conclusions: The identified functional disorders of the heart in the comorbid course of coronary heart disease and obesity can be used for early diagnosis of cardiovascular complications in such patients and for the development of adequate therapeutic schemes.Документ Key directions in the organization of health care and prevention actions to reduce mortality from cardiovascular diseases in Ukraine(Graz, Austria, 2020) Mishchenko, M.; Shevchenko, A.; Mishchenko, A.Документ Medical and social evaluation of quality of life of children with disabilities(Харківський Регіональний Інститут Проблем Громадської Охорони Здоров'я, 2020) Mishchenko, A. N.Вased on the results of original research, the levels of quality of life of different classification groups of children with disabilities in Kharkiv-city and Kharkiv region in terms of providing them with medical and social assistance were analyzed.Документ National trends in morbidity and mortality from circulatory system and cerebrovascular diseases and strokes(Wydawnictwo Aluna, 2022) Ognev, Victor A.; Mishchenko, Marina M.; Mishchenko, Alexander N.; Trehub, Pavlo O.The aim: To determine national trends in morbidity and mortality from diseases of the circulatory system, cerebrovascular diseases and strokes. Materials and methods: Data from official sources of statistical information of Ukraine were used and systematic analysis and generalization of the obtained data was performed and trends in morbidity and mortality from diseases of the circulatory system, cerebrovascular diseases and strokes in Ukraine were calculated. Results: Were found tendencies to decrease of national levels of prevalence and primary morbidity in Ukraine for DCS (-16.3 % and -28.0 %), CVD (-22.8 % and 24.1 %) and strokes (-12.2 %) with significant trends (+83.9 %) of increase in primary incidence of strokes in 2010-2017 with fairly high and threatening levels for 2017 (respectively 22199563, 2521601 and 96978 – prevalence and 1725137, 290557 and 96978 – primary incidence). National levels of reduction of deaths from DCS in Ukraine from 440369 (2013) to 389348 (2019) with a trend of -11.6 % and a decrease in mortality due to CVD from 94267 (2013) to 76232 (2019) with a trend -19.1 % were found. Conclusions: The trends to reduce of national prevalence, primary morbidity and mortality rates in Ukraine for DCS, CVD and stroke are fully consistent with other global trends of reduction of these levels among world countries.Документ Prevalence of anxiety and depression in patients after brain stroke(Державна наукова установа "Науково-практичний центр профілактичної та клінічної медицини" Державного управління справами, 2023) Mishchenko, Marina M.; Ognev, Victor A.; Mishchenko, Olexander M.; Ponomaryov, Volodymyr I.Introduction. WHO experts point to the urgency of the problem of cardiovascular diseases, among which the first places are occupied by cerebral strokes (CS). With CS there are significant changes in the socio-psychological activities of the patient and the personal psychological characteristics of the individual. Such changes provoke the development of certain psychological disorders, which are characterized by an oppressed state, a persistent depressive state, the development of anxieties and fears, obsessive fixation on the transferred disease and worries about one’s future, etc. That’s why, the study of psychological characteristics of patients after СS is a relevant problem of great importance, which determined the direction of our research. The aim: To investigate and analyze the prevalence of anxiety and depression in patients after CS.Materials and methods. Conducted a randomized controlled single- center prospective study case-control, which is based on the analysis of the results of psychological study of 300 patients after CS (main group) and 200 people of the control group for the period 2022-2023. The studied groups were randomized by age and gender. To study the psychological characteristics of patients after CS, we used two methods of diagnosing psychological conditions: the self-esteem scale of Spielberg Ch. D. and Khanin Yu.L. and the depression scale of Balashova T. I.Results. Probably (p<0.001) among the subjects of the main group compared to the control, a significantly higher frequency of occurrence of medium and moderate levels of reactive anxiety (RA), personal anxiety (PA) and levels of depression (LD) was determined, in contrast to low, which was more often detected among the persons control group. It was determined that the levels of RA, PA and depression were reliably significantly higher in the main group compared to the control group and characterized the presence of fairly significant moderate anxiety and mild depression of situational or neurotic origin.Conclusion. It has been proven probable (p<0.001) confirmation of the influence of an CS on the development of high levels of anxiety (both RA and PA) and depression.Документ Prevention of cardiovascular disasters at operators of control panels of nuclear power plants(ГО "Львівська медична спільнота", 2020) Mishchenko, M. M.; Shevchenko, A. S.; Mishchenko, A. N.; Shevchenko, V. V.The electrical equipment of Ukrainian power plants is worn out, but not replaced with new ones, but repaired and modernized, which increases the possibility of emergencies. For the safe operation of NPPs, the number of parameters for monitoring the operation of this equipment is increased. These two factors increase the These two factors increase the burden on NPP's control panels operators. This increases the risk of cardiovascular accidents (strokes and heart attacks) of operators, despite their careful professional selection and constant medical examinations. Measures of medical control of operability of operators during their work are offered.Документ Risk factors of ischemic stroke and their interconnection(Perfect Publishing, 2020) Mishchenko, Marina Mikhailovna; Shevchenko, Alexander Sergeevich; Mishchenko, Alexander NikolaevichCardiovascular disease is the leading cause of premature death. In turn, among cardiovascular diseases, the leader is ischemic stroke. The main risk factors for ischemic strokes (behavioral, physiological, genetic, environmental) act synergistically and are pathogenetically interconnected.Документ Study of risk factors of cerebral strokes according to the data of regional registers and national services of medical statistics(CPN Publishing Group, 2020) Ognev, Victor Andreevich; Mishchenko, Marina Mikhailovna; Mishchenko, Alexander Nikolaevich; Shevchenko, Alexander SergeevichUsing the registers of patients with cerebral stroke, the frequency of occurrence of risk factors in various populations was studied. A comparison of the averaged data was carried out, followed by a comparison with the data of national and international prospective studies of stroke risk factors. The main (“significant”) risk factors remain unchanged in any population. Their corrections are attributed to 2/3 of the success in reducing mortality from strokes (1/3 refers to the success of therapy).Документ The identification of risk factors of cardiovascular disasters and monitoring of actions of medical staff(Cognum Publishing House, 2020) Mishchenko, Maryna Mykhailovna; Mishchenko, Alexander Nykolaevich; Shevchenko, Alexander SergeevichSelf-identification of the first signs of cardiovascular disease and control of risk factors are an important part of their prevention. Such a strategy is also a component in the formation of the valeological competence of people without a medical education, and increases the chances of survival in emergencies. However, the organization of medical care and the financing of health programs play a leading role in reducing mortality and disability. However, the organization of medical care and the financing of health programs play a leading role in reducing mortality and disability.Документ World strategy on tobacco control and mortality from cardiovascular diseases in Ukraine(SSPG Publish, 2020) Ognev, Victor Andreevich; Mishchenko, Maryna Mykhailovna; Mishchenko, Alexander Nykolaevich; Shevchenko, Alexander SergeevichSince 2003, Ukraine has been participating in the WHO Framework Convention on Tobacco Control and has achieved a slight decrease in the number of smokers according to state medical statistics. The decrease in mortality from cerebral stroke and coronary heart disease over the past 10 years is associated with this fact.
