    Impact of prooxidant-antioxidant imbalance on the biological age and the rate of aging in arterial hypertension with type 2 diabetes mellitus

    Purpose:to study the state of oxidative-antioxidative balance and its influence on the biological age (BA) and the features of aging in patients with a comorbid course of arterial hypertension (AH) and type 2 diabetes mellitus (T2DM). Materials and methods.In 96 patients with II stage AH and T2DM and 40 patients with isolated II stage AH (IAH), a BA, rate of aging (according to the method of V. P. Voytenko et al.) and oxidative stress (OS) indexes (activity of glutathione peroxidase, serum levels of sulfhydryl groups, malonic dialdehyde, 8-hydroxy-2-deoxyguanosine) were investigated. Results. In IAH patients, physiological aging (PhA) was detected in 12.5 %, delayed type of aging (DTA) – in 55.0 %, accelerated type of aging (ATA) – in 32.5 %. In AH and T2DM, PhA was in 9.4 %, DTA – in 31.3 %, ATA – in 59.4 % of patients. Only 5 patients (12.5 %) with IAH and 3 patients (3.13 %) with AH and T2DM had no signs of OS. The patients with ATA in comorbid pathology were characterized by a significant activation of oxidative and stimulation of antioxidant systems compared to those with DTA, which was less common among IAH patients. The presence of correlations between the antioxidant system indicators and age parameters characterizing the degree of aging was found. Conclusions. OS, which is one of the significant factors resulting in premature aging, was more enhanced in AH and T2DM than in IAH. OS manifestations were more significant in ATA regardless of nosology, but were more pronounced in AH and T2DM. Therefore, OS evaluation together with age-related characteristics can be used both to assess an organism state and as an integral indicator characterizing the effectiveness of therapeutic and preventive measures.

    Effects of biological age and aging rates on the expression of risk factors in patients with arterial hypertension and type 2 diabetes mellitus

    Objective: biological age has been given a large role in determining the significance and contribution to the general concept of risk factors in recent years. The relevance of determining the rate of aging is also due to the fact that the rate of aging may have a real prognostic value for assessing the health of both an individual and groups exposed to one or another risk factor. The aimof the present paper was to study the biological age, to assess the rate of aging in persons with high cardiovascular risk (CVR), as well as to carry out search for possible correlations of agerelated indices of aging and some risk factors. Methods: During 2016-2018, 96 patients (42 men and 54 women), whose mean age was 62.66 ± 4.21 years, with AH stage II (disease duration of 10.2 ± 3.7 years) and DM2T, disease duration of 4.1 ± 2.4 years) were included in the study. The biological age and rate of aging was investigated by the method of V.P. Voitenko et al. Results: Physiological aging was observed in 9.38% (9/96) patients, in 31.25% (30/96) therewas a delayed rate of aging, in 59.38% (57/96) -an accelerated rate of aging (ARA). ARA was characterized by a more pronounced body mass index, fasting blood glucose level, insulin resistance degree. In men, ARA was observed 2 times more frequently than in women. The most significant correlations between the indices characterizing the biological age and the rate of aging were recorded with systolic, diastolic and pulse arterial pressure. Conclusion: Thus, the complex ofanthropometric and metabolic disorders can act as an initiating syndrome of accelerated aging, thereby aggravating the existing cardiovascular risk.

    Age-associated features of oxidative stress as marker of vascular aging in comorbid course of hypertension and type 2 diabetes mellitus

    Aim: To evaluate activity of oxidative stress (OS) as marker of vascular aging in different age groups of patients with combined course of arterial hypertension (HT) and type 2 diabetes mellitus (T2DM). Methods: 126 patients (average age 57.8 ±6.2 years) with stage II HT and compensated T2DM were divided into 2 subgroups: 2a (n= 59) - aged 45-60 years; 2b (n= 97) - aged 61-75 years; 30 patients with isolated stage II HT (comparison group), 20 practically healthy individuals (control group). The activity of antioxidative [glutathione peroxidase, sulfhydryl groups (SH-groups)] and oxidative [malonic dialdehyde (MDA)], 8-hydroxy-2-deoxyguanosine (8-OH-dG) systems in blood serum, were studied. Results: A significant increase in MDA levels (P< 0.05) and SH-groups (P< 0.05) compared with healthy volunteers was observed. Patients in 2b group had lower MDA values than in 2a (6.25 ±0.33 μmol/L, 7.07 ±0.44 μmol/L, respectively, P> 0.05). In the 2b group, in comparison with 2a patients, a decrease in thiol status was observed (P> 0.05). The level of 8-OH-dG was increased in patients with HT and T2DM, but there was also an age-associated increase in the average 8-OH-dG in the 2b group. Conclusion: The age-associated changes in the OS in comorbid course of HT and T2DM did not have significant differences. Nevertheless, the presence of correlations between various indexes that are included in the concept of “vascular aging” and indicators of oxidant-antioxidant systems in different age groups allows us to make an assumption about the significant influence of the oxidative status on the status of vascular age, especially in the older age group persons.

    The effect of statin therapy on oxidative stress indices in patients with arterial hypertension and type 2 diabetes mellitus

    Objective: to study the effect of statin therapy on the oxidative and antioxidant systems parameters in patients with arterial hypertension (AH) and cocomitant type 2 diabetes mellitus (DM2T). Materials and methods. 126 patients (55 males and 71 females, average age was 57.8 ± 6.2 years) with AH stage II and compensated DM2T were divided into 2 groups: the 1 group – with AH and DM2T (n = 69), who were constantly taking statins (rosuvastatin 10 mg/day or atorvastatin 20 mg/day) for at least 1 year; the 2 group – patients with AH and DM2T (n = 57) who did not take statins. The control group included 20 healthy volunteers. The parameters of lipid and carbohydrate metabolism, the degree of insulin resistance (HOMA-IR), the state of the oxidant system (malonic dialdehyde level –MDA), the antioxidant system (the activity of glutathione peroxidase (GPO) and the level of sulfhydryl groups-SH-groups) were evaluated. The statistics was carried out using the Statistica software package, version 8.0. Results.In the 1st group only the levels of LDL cholesterol significantly differed from the control group (P < 0.05). In the 2nd group the levels of total cholesterol and LDL cholesterol were expected to be higher than in the 1st group (P < 0.05) and the control group (P < 0.05). Despite the higher level of fasting glucose in the 2nd group than in the 1st group, HOMA-IR in the 1st group was higher than in the 2nd group (7.48 ± 1.76 and 7.17 ± 1.54, respectively, P > 0.05). In the 1st group in comparison with the 2nd group unreliable increase in GPO and SH-groups levels on the background of the increase in MDA levels (P > 0.05) were observed. Conclusions. The use of statins in low doses in AH combined with DM2T was accompanied by a nonsignificant improvement of antioxidant protection parameters on the background of increased insulin resistance and increased activity of lipid peroxidation.
