Огляд літератури з питань лікування гострої емпієми плеври

Abstract

Кількість хворих із гострою деструкцією легень становить понад 68,4 % від загальної кількості запальних захворювань грудної порожнини та середостіння. Гострі неспецифічні гнійнодеструктивні захворювання легень та плеври (ГДЗЛП) відносяться до розряду важкої патології в хірургії. Незважаючи на стрімкий розвиток торакальної хірургії рівень кількості ускладнень та летальності не знижується, а її ускладнення призводять до інвалідизації. Для поліпшення якості лікування пацієнтів при ГДЗЛП та їх хірургічних ускладнень важливе практичне значення має об’єктивне формування груп ризику розвитку ускладнень та прогнозування динаміки перебігу. Успішне вирішення цієї проблеми багато в чому пов’язане з високоінформативною ранньою діагностикою, достовірним визначенням ступеня тяжкості захворювання, створенням різноспрямованої класифікації запальних захворювань легень та плеври. Ці питання не можуть вважатися остаточно вирішеними та вимагають подальшої розробки. Все це диктує пошук нових ефективних методів лікування названої патології і свідчить про актуальність теми. Висвітлені відомості вказують на необхідність вдосконалення хірургічної тактики у хворих на емпієму плеври.

Acute non-specific purulent-destructive lung and pleura diseases belong to the category of severe pathology in surgery. In spite of dramatic development of thoracic surgery the level of complications and mortality does not decrease and its complications lead to invalidism. The number of patients with acute lung destruction is more than 68.4 % of the total number of inflammatory diseases of the chest cavity and mediastinum. Acute lung pathology becomes chronic in 11–40 %, leading to temporary disability in 30 40 % and permanent disability in 7.1–9.7 %. Mortality in purulent-destructive diseases of the lungs and pleura is 7.2–28.3 %, and in its gangrenous forms – 23.4. The proportion of complicated forms of abscessing pneumonia has increased from 15.8 % to 43.6 % in recent years. In order to improve the quality of treatment of patients with purulent-destructive diseases of the lungs and pleura and their surgical complications, it is of great practical importance to objectively form risk groups for complications and predict the dynamics of the course. Successful solution of this problem is largely related to highly informative early diagnosis, reliable determination of the severity of the disease, creation of a multidirectional classification of inflammatory diseases of the lungs and pleura. To improve the quality of treatment of patients in purulent-destructive lung and pleura diseases and their surgical complications an important role is played by objective formation of risk groups of developing complications and forecasting of the dynamics of the disease. Successful solution of the problem is related mostly to highly informative diagnostics, reliable determination of the severity of the disease, as well as creation of multidirectional classification of inflammatory lung and pleura diseases. These issues cannot be considered to be completely solved and require further study. Everything mentioned above dictates the search of new effective methods of the treatment of the mentioned pathology and proves the relevance of the theme. The outlined information highlights the necessity of improvement of surgical tactics in patients with pleural empyema.
